The U.S. Food and Drug Administration (FDA) announced that it has launched web versions of four forms in its CTP Portal Next Generation (CTP Portal NextGen) for applicants submitting or amending premarket tobacco product applications (PMTA) and Substantial Equivalence (SE) reports.
These web-based forms (Form FDA 4057, Form FDA 4057a, Form FDA 3965, and Form FDA 3965a) allow applicants to fill out, validate, and submit their applications directly through the portal — with features like step-by-step instructions, multi-user collaboration, saving drafts, and real-time validation to ensure all required fields are completed.
For those who prefer not to use the web-based version, PDF versions of these forms remain available. However, the FDA published updated PDF forms (correcting minor technical issues) in June 2025, and from January 2, 2026, submissions using PDF versions must use the latest version — otherwise the FDA may refuse to accept the application.
The move is part of FDA’s efforts to enhance efficiency, effectiveness, and transparency in the tobacco-product application review process.
